How is a metacarpal neck (Boxer's) fracture treated? (Surgical Reduction Maneuver)

Non-surgical:

A bulky splint covering the wrist and hand for 4-6 weeks with loss of hand function for this duration and a permanent deformity of the hand.

Surgical:

  1. Reduction maneuver used by the surgeon to correct fracture alignment.
  2. Surgeon then drills pins across the fracture to hold it in place.
  3. The surgeon then applies a bulky splint, to be worn for 4-6 weeks.


This splint simply replicates this surgical reduction technique and holds the bone in alignment until the fracture is healed without surgical pins or bulky splint.

How to Apply the Metacarpal Neck (Boxer) Fracture Splint

  1. Slip a staggered splint over your ring and little fingers. (There is a left and right kit, so be certain you have chosen the correct kit for your injury.)
  2. The splint should feel snug or loose, but NOT TIGHT.
  3. Apply pads between the splint and skin until snug, starting with the near palm side of the little finger, as this adds pressure to help align the fracture. There are 2 types of pads. The thin/thick long pads can be wrapped around the close end of the little finger portion of the brace, enhancing the reduction force on the fracture.
  4. Apply the wrist strap and pass the tether through the far end of the brace on the larger finger, bend the knuckle ( MCP joint ) 90 degrees and attach the Velcro tether to itself.

Adjustments

1)     The pads and/or splints can be exchanged for a tighter or looser fit as the swelling increases or decreases, creating a more comfortable and safer fit.

 

2)     The tether length can be adjusted to change the flexion of the MCP joint. The ideal flexion is 90 degrees, but it can be kept at less flexion for a few days during acute swelling. The tether can be released temporarily to allow activities such as typing or reaching into a purse/ pocket.
